Digital Transformation in Australian Government: Why Most Projects Fail

The Root Cause Is Not Technology

Government digital transformation failures are almost never caused by technology not working. They are caused by: lack of a single accountable technology leader with actual decision-making authority; procurement processes that select vendors on price and compliance rather than capability; waterfall delivery models producing nothing for 18-24 months; and insufficient change management resulting in low adoption of successfully delivered systems.

The Governance Problem

The most consistent pattern in failed government projects is a governance structure designed to distribute accountability rather than concentrate it. When 14 stakeholders have approval rights over technical decisions, every decision is delayed by weeks. The fix is simple but politically difficult: appoint a single accountable technology leader with genuine authority to make decisions within defined boundaries.

Agile in Government: The Reality

Many agencies responded to waterfall failures by mandating Agile, then continued applying Agile methods to procurement and governance structures designed for waterfall delivery. Using two-week sprints to build to a fixed scope specified 18 months ago is not Agile — it is waterfall with more meetings. True agile government delivery requires accepting that scope will change as learning occurs.

The Case for External Technical Leadership

The APS has deep domain expertise and genuine commitment to public service. What it frequently lacks is the commercial technology delivery experience that comes from shipping products at scale in the private sector. External technical leadership brings private sector delivery models into government programmes while operating within APS governance structures.
